Office Hours: Monday 3:30 - 6:30 :: Tuesday - Friday 11:30 - 7:30 Occasional Saturday appointments available. Appointments Appointments are made by calling the office during regular business hours. Appointments are generally scheduled within two weeks, and more often than not much earlier. Emergency appointments are scheduled immediately. It is important that you let the staff know you need an emergency appointment. Cancellation Policy Cancellations should be made 24 hours in advance of an appointment, except of course, in the event of an emergency. A cancellation fee of $40.00- $100.00 will be charged for missed appointments, which is generally not reimbursed by insurance companies.* Release of Information Information to other agencies, health professionals, and attorneys. will be done only when the other party makes a request in writing and is accompanied by a properly executed release of information form which can be obtained from the office. AFTER OFFICE HOURS CALL THE OFFICE AND FOLLOW THE PROMPTS FOR AN EMERGENCY DOCTOR ON CALL. If you cannot wait please go to the Emergency Room of the closest hospital and ask them to call the doctor. Confidentiality Due to the need to maintain strict confidentiality, information will not be given to anyone without the patient’s approval and after consultation with Dr. Thampy. In an emergency this approval may be dispensed with by Dr. Thampy. If you intend to have a friend or relative accompany you to an appointment please let us know in advance. Psychiatric Treatment Generally the initial visit is about 45-60 minutes long, and involves making a diagnosis and starting a treatment plan. The interview consists of obtaining a psychosocial and medical background history, inventory of symptoms, and assessment of memory and other intellectual functions. Subsequent sessions range from 10-60 minutes, depending on the treatment prescribed. Frequency of follow up visits may vary from weekly to every three months. All of this will be discussed with you at the end of the initial evaluation. Prescription Refills Try not to run out of medication. If you need a refill or prescription before your next appointment, call the office and let us know what medication, dosage, and amount you need. Have your bottles when you do this and be sure to leave the telephone number of your pharmacy. Some prescriptions such as stimulants and narcotics, cannot be called to the pharmacy, and require a written prescription. Medication Side-Effects All medications have side effects. Some are very common, others quite rare. We will provide you information about the more common side effects. However, it is very important that you notify us if any unusual symptoms develop while you are taking medications. It is critical that you keep us informed about all medications prescribed by other health professionals, over the counter pills, vitamins, health foods/supplements that you maybe taking. Medical Affidavits If you need a disability statement, insurance forms completed and so forth please inform the staff before your appointment so they can prepare them in a timely manner. To obtain a work release you must contact the office on the day you need the statement. Insurance It is essential that you provide us with up to date insurance information, so the office can take the necessary steps to obtain authorization. If your insurance company does not authorize the treatment, you will be responsible for the bill. Generally we will bill the insurance company on your behalf. You will be billed for your portion of the balance. You will be sent a statement on a regular basis to keep you informed. It is not necessarily a statement of your balance. Fee Schedule An updated fee schedule is available on request. Laboratory Testing If any lab testing is ordered, please check with the doctor or the laboratory whether any special procedure is to be followed. * Does not apply to Title IX (Medicaid) recipients |
